Dear User, I wish to inform you that the GDRP (General Data Protection Regulation (EU) 2016/679) states that the protection of people’s personal data is a fundamental right.

I, Francesca Moresi, in the capacity of “data controller” and according to article n.13 of the GDPR, inform you that:

DATA CATEGORIES:  I will collect personal data that will be inserted in the form.

SOURCES OF PERSONAL DATA: the personal data that I hold are collected directly from the form you when submitting the it.

HOW I USE DATA: the collection of your data through the Contact Form relies on your consent and the purpose is to reply to your queries through the form.

DATA RECIPIENT: I only will responsible for handling and holding your data. There may be occasions when I need to share the personal information I process about you with third parties. When I do so, I comply with all aspects of the Data Protection Act 1998 (DPA).

If you are claiming the cost of your sessions through your insurance company, your insurance company may request details of your treatment and progress from me in order to authorise further funding for your treatment. Under these circumstances, I will share the minimum amount of information necessary with your insurance company. I will also need to share some of your persola data (such as name, address and date of birth) in order to invoice your insurance company for the cost of our sessions.

In order for me to provide you with the best treatment, I may need to seek advice from other mental health professionals. All psychotherapists consult with a Clinical Supervisor on a regular basis for the duration of their career to ensure best practice is maintained. If I do need to talk with my clinical supervisor or colleagues about your case in order to help you, this will be done with your best interests in mind. Your identity will always be kept confidential so your name will never be disclosed. Furthermore, the content of what we discuss will also be treated with the same level of confidentiality and the same exceptions to confidentiality will apply to other professionals as well.

There are three situations where I would share your information with third parties, without your consent:

Court Order

If I am required to disclose data about you under a Court Order

Child Protection

If I am concerned about the welfare of a child, i.e., where there are child protection issues

Risk to self or others

Where there is an imminent risk of harm to yourself or others, i.e., you have expressed an intent to kill yourself, or to kill someone else, imminently.

As per the BABCP Standards of Conduct, Performance and Ethics, I must take appropriate action to protect the rights of children and vulnerable adults if I believe they are at risk, including following national and local policies.

INTERNATIONAL TRANSFER OF DATA: data will not be internationally transferred .

DATA RETENTION:

Unless a longer retention period is required or permitted by law, I only will hold your Data on my systems for the period necessary to fulfill the purposes outlined in this privacy policy or until you request the Data to be deleted (Purpose limitation principle).  We will periodically review the data retention with the purpose of their collection.

YOUR RIGHTS

You always have the right to access, correct, erase, restrict our use of your Data, the right to data portability and the right to object.

MANDATORY DATA PROVISION

I wish to inform you that data provision is discretionary for some fields of the form whilst for others it is mandatory (marked with *). Not giving your consent for data provision will result in NOT sending the form.

MANDATORY CONSENT

Giving your consent to transfer your data through ticking the apposite box is discretional, but it is mandatory to submit your form.

DATA PROCESSING MODALITY

The personal data provided by you will be collected in accordance with this privacy policy and the privacy required by the owner’s profession. Data will be processed through electronic and paper format or any other apposite format, according to the privacy measures of the GDPR.
